Immunization Requirements : California School Immunization Records ( CDPH286 white paper form ) must exist kept and be available at the time of Community Intendance Licensing visits. Records must be completed past child care staff and parents must provide immunization information. Personal Beliefs Exemption: Information technology is no longer sufficient to take a parent sign the back of the California School Immunization Record as a personal belief exemption. A Personal Beliefs Exemption to Required Immunizations (CDPH 8262) must be on file instead.
Parents will no longer be able to submit a personal belief exemption afterwards January one, 2016.
Personal belief exemption forms submitted before Jan 1, 2016 are valid until the child changes schools or continues to the adjacent "grade span."

Incidental Medical Services: Licensed Family Kid Care Homes and Child Intendance Centers may choose to offer any wellness care related services (nebulizer, Epi-pen Jr, etc). Whatever health intendance services provided following doc'due south orders are Incidental Medical Services (IMS). Wellness intendance services such as administrating prescription medication that does not require special training and all non-prescription medications are not considered IMS.
NO Incidental Medical Services (including storing an Epi-pen Jr or nebulizer) can accept place in any licensed facility unless there is an canonical, written Incidental Medical Services Plan on file with CCL.

Healthy Schools Human activity (Child Care Centers): As of January i, 2015 Kid Care Centers must develop an Integrated Pest Direction (IPM ) Plan and send pesticide utilize reports to the Department of Pesticide Regulation (DPR)
